Please indicate your full name, signature, (maiden name if applicable), graduation or discharge year, telephone number, date of birth, and the address where you would like it mailed. Proof of identification is required by either picture I.D. or a notarized letter. Pick ups are also available between 8:00am-12:00pm or 1:00pm-3:00pm.
