1) Your full name, (maiden name if applicable)
2) Proof of identification (PHOTO ID)
3) Graduation or discharge year
4) Telephone number
5) Date of birth
6) The address where you would like it mailed.
During July & August all transcripts will be done twice a week starting July 11. We will get them to you as soon as possible.
